INSTRUCTIONS FOR HOW TO SUBMIT THIS FORM

This application must be accompanied by the rental fee and deposit (TWO CHECKS MUST BE MADE OUT):

1. A nonrefundable fee/clubhouse rent of $250 for a full day (from 8:00 AM through 10:00 PM), and

2. A $200.00 refundable cleaning/damage/violation deposit fee.

Both checks must be made payable to Eden Shores Homeowners Association. The check for for clubhouse rent will be deposited while the check for cleaning/damage/violation deposit will be held until the premises have been checked out after its use.

COMPLETE AND MAIL THIS ENTIRE FORM, TOGETHER WITH YOUR TWO CHECKS, TO:
